Anguish Incarnate is a Main Quest in Diablo 4. Anguish Incarnate can be found at Tarsarak. Main Quests are mandatory missions that players must complete in order to finish the storyline and reach the next difficulty level. Main Quests must be done sequentially and are unlocked only after completing the previous Main Quest/s.

Diablo IV Anguish Incarnate Walkthrough
This main quest is pretty straightforward where you'll need to defeat the boss to complete it. The "Anguish Incarnate" quest in Diablo 4 immediately triggers after completing "A Meeting of the Minds".
Andariel first appears chained on an arch-like pillar, this boss will constantly spawn statues around the arena that will conjure different effects and attacks, if you can, try to destroy these statues to prevent them from overwhelming you with attacks but if you can soak up enough damage if you have enough health, focus on Andariel. Most of Andariel's attacks at first will compose of AoE-based attacks and you can easily identify how to avoid being hit by simply looking at how it is formed on the ground.
Inflicting enough damage will cause Andariel to retreat and shoot out projectiles before the boss reappears. It's easy to avoid the projectiles by dodging or simply running. Once you've applied about 40-45% damage to Andariel's health, the boss will fall to the ground and spider-like limbs will emerge from its back, making it more aggressive with its movements.
Andariel will now be able to chase you and it has a longer reach with its sweeping attacks, a good tactic is to dash and attack from behind or the sides to avoid getting hit in the front. Repeat this process while watching out for the runes that spawn around. Successfully defeating this boss yields items that drop from the boss, as well as XP and Gold.
