Apex of Misery is a Main Quest in Diablo 4. Apex of Misery can be found at Scosglen. Main Quests are mandatory missions that players must complete in order to finish the storyline and reach the next difficulty level. Main Quests must be done sequentially and are unlocked only after completing the previous Main Quest/s.

Diablo IV Apex of Misery Walkthrough
After clearing the fog at the end of Harrowed Lament, you can now make your way towards Solitude to find and confront Airidah. Follow the path northeast to get to the quest marker, where you'll find a Healing Well and encounter several Banshees and Vengeful Spirits. If you'd like, you can also interact with an Altar of Lilith on the northwest path branching off from the path you need to take to get to your quest.
On the way to your target, you'll find and have to destroy two more Risen Remains. These Risen Remains are tethered to two enemies nearby and cannot be damaged until the they have been defeated. The enemies in the area include Banshees, Vengeful Spirits, and Wildwood. After the two tethered enemies have been defeated, you can destroy the Risen Remains and run away from the rest of the mobs. Follow the path to the northwest and destroy the Corrupted Roots in your way to progress.
You'll hear Airidah's voice beckon you over as you get closer to Solitude. At the top, you'll find a Healing Well and you can finally speak to Airidah to find out what she's up to. She explains that the tragedies befalling her people now is necessary to prepare them for the coming of Hell. You'll then have to fight her in order to earn the right to know what Airidah gave Lilith in exchange for her newfound power.
The boss fight against Airidah, Keeper of the Dead begins shortly after your conversation ends. You'll notice three triangles spread evenly across her health bar at the top; these triangles will come in later. Airidah is a spell caster and, for the first phase of her fight, will mainly cast a teal-hued AOE spell that rains from above. This spell can be countered using any ability that negates ranged attacks. Airidah, in addition, may also turn invisible and reposition herself farther away from you before casting her spells. She isn't too hard to track when she does this and a puff of smoke appears where she will emerge from.
When her health bar reaches one of the triangle indicators, Airidah teleports to the middle of the arena and becomes invulnerable. Similar to the Risen Remains you encountered earlier in the quest, Airidah tethers herself to two Wraiths that must be defeated before she can be damaged again. The Wraiths themselves will not attack, but Airidah will continue to cast spells from her bubble.
After the two Wraiths have been defeated, Airidah can be damaged and will return to her first phase, attacking you with various spells. This time, she has 2 additional spells to use. One, if you're playing a melee character, will knock you back dealing moderate damage. For the second spell, she summons two small sand tornadoes at either side of her that move outwards and explode on impact or after a short period of time. Be patient and run away from these tornadoes as they deal a large amount of damage if they hit you.
Take her down to the next triangle indicator and she'll become invulnerable once again and summon three Wraiths. Defeat the Wraiths like last time and Airidah will become vulnerable. Her skills become more powerful during this phase, with her teal AOE spell having additional hits. Keep damaging her until her health bar reaches the final triangle marker, and she'll summon four Wraiths this time. Same deal, defeat the Wraiths and finish the rest of her health bar to finish the fight.
After defeating Airidah, she reveals that she told Lilith how to break the wards keeping the demon Astaroth imprisoned. At the end of the conversation, she collapses by your feet and Apex of Misery concludes. You'll be rewarded with 13,000 XP and 1,540 Gold, and the next quest, Parting Embers, will begin.
