Penitent Cairns is a Dungeon in Diablo 4. Penitent Cairns is an area avoided by townsfolk because spirits consider it a sacred place. The Resurrected Malice is the final boss of the dungeon. Penitent Cairns can be found at Scosglen.

Penitent Cairns can be completed by doing:

- Travel to the Caves of Atonement
- Survive the enemy attack: 30 seconds.

- Defeat the Resurrected Malice.

Diablo 4 Penitent Cairns Walkthrough
Upon entering the dungeon, you'll find yourself within the Mournful Hollows. You are first tasked with freeing seven prisoners. Make your way to the first prisoner by following the main path, you won't come across any enemies as you make your way to the first prisoner so you can go at your own pace. As you continue, you will encounter a variety of enemies like Stinging Swarm, Banshee, and Thorn Beast, to name a few. This dungeon has tight and narrow corridors and getting overwhelmed by enemies can lead to your death. Make sure you have abilities great with crowd control equipped. Some prisoners will be guarded by enemies, and if they die, you still need to lay them to rest.
Once you've freed all seven prisoners, make your way to the Caves of Atonement. You'll then enter a room where you have to survive waves of enemies for 30 seconds in order to dispel the barrier that's blocking your path. Once you reach the Cave of Atonement, you'll be tasked with finding the Stone Carving and returning it to the Pedestal. You'll encounter a number of elite monsters as you search for the Stone Carving, so be careful and try to slay them as you proceed to not get swarmed by them. Continue following the main path and you'll see the Stone Carving on your map marked with the white icon. There will also be a chest near the Stone Carving, make sure to loot the chest. The Stone Carving will be guarded by multiple Vengeful Spirits, Bashees, and Shambling Corpses.
Now make your way to the Pedestal and place the Stone Carving to dispel the barrier blocking the room. There's a Healing Well beside the entrance so you can recover some HP if needed. Enter the room and this is where you'll encounter the dungeon boss, Resurrected Malice. This boss doesn't have a lot of attacks, but all of them are long-ranged attacks. It has the ability to create a spirit ball and throws it as an attack, it can sometimes split into three after a short period of time. This attack is easily telegraphed as you will see when it charges up to create the spirit ball. It can also summon Vengeful Spirits into the battlefield to aid in battle. You need to quickly take care of them to not get overwhelmed with attacks and to focus mainly on the boss, because if you don't, the Resurrected Malice can summon more and as they stack up, it can lead to your death. Once the Resurrected Malice is defeated, the dungeon is completed.
