Poisoned Hearts is a Side Quest in Diablo 4. Poisoned Hearts can be found in Zabinzet at the Diseased Embankment at the Fethis Wetlands in Hawezar. Side Quests are optional missions that players can complete in order to obtain different rewards. Players are usually rewarded XP, and Gold depending on your level. Completing Side Quests in a specific region will also add to the Region Renown. Side Quests can be acquired by encountering various events or interacting with different NPCs.

Poisoned Hearts Walkthrough
After completing the Side Quest A Price to Pay, this Quest will automatically be activated and your objective will be highlighted accordingly. In case you left this next Quest for later, your objective will be in the Diseased Embankment, near the entrance to Blind Burrows in the Fethis Wetlands.
- Speak with Vera
Vera will be bent over near the boiling pot you used during the previous Quest. Approach her and ask, "Vera, what are you doing here?" Seems her dislike for her own daughter goes far beyond hatred.
- Collect Aneta's Keepsake
Aneta's Keepsake should be just beside the pot. As the kind adventurer you are, you'll endeavor to bring it to her.
- Find Aneta
The next objective area indicated by a blue circle will be southeast of where you currently are, leading to the Forsaken Cost. Place an objective marker on your map for the fastest path there, or teleport to the nearest Waypoint first if you've unlocked them. Aneta should be in the southwest area of the circle, in Gulya's Hovel.
- Extinguish the Strange Braziers: 0/3
She seems to be in the middle of a ritual or something and it may have gone awry.
- Speak with Aneta
Approach Aneta and ask, "I found your necklace. Are you alright?" It seems you meddled with a ritual that was supposed to help Aneta. Nevertheless, it seems it wasn't bearing any fruit.
- Speak with Gulyas
Talk to Gulyas now and ask her, "What must we do?" You'll have to help them use ancient magic to help Aneta.
- Help Aneta gather rotten livers: 0/4
The next objective area indicated by the blue circle should just be to the east, at The Rustwash. Aneta will point out when you're near one and automatically go to it. Aneta will gather it whenever you find one, while you'll have to fend off Enemies whose attention you've attracted while she does so. The exact locations aren't set, but just keep an eye out for anything that either looks like a dead snake or a dead human corpse.
- Enter the Hidden Glade
After you've collected enough, you'll be asked to get to the Hidden Glade, which is a Cellar to the east, in the Blackguard Bog. You can find the Cellar on our Interactive Map here: [Diablo 4 Map Link].
- Follow Aneta
Inside, follow Aneta along the singular path. In the spacious cavern, you'll start the ritual to help Aneta.
- Speak with Gulyas
Approach Gulyas and declare, "We are ready." to begin the ritual. It seems it'll entail killing Enemies that Gulyas will draw near.
- Fill the Blood Altar
All manner of beasts and insects and slithering creatures will run into the red circle. Just keep killing them, using any abilities you have that hit multiple Enemies at once until the red bar on the right gets filled up.
- Protect Aneta from the villagers
Something was bound to happen with everything going so smoothly. Easily take care of the villagers who dare to disturb the ritual. One of them turns on the mother, who despite everything Aneta still protects and runs after.
- Follow Aneta
Exit out of the Cellar and run after Aneta and her mother. The objective marker should lead you west, to the Manhunter's Hollow.
- Speak with Aneta
Approach the grieving Aneta and tell her, "Aneta, I'm so sorry." Strange as it sounds, all the voices in Aneta's head have quieted with the death of her mother.
- Return to Gulyas' Hovel
Follow the pair back to Gulyas' base to the west. You'll find Aneta gone but she's left you a letter. Head to the table to the northwest and interact with it.
- Read Aneta's Letter
She's left you her keepsake, to follow the call of the voices in her head. This will complete the quest.
