Prying the Eye is a Main Quest in Diablo 4. Prying the Eye can be found at Kehjistan. Main Quests are mandatory missions that players must complete in order to finish the storyline and reach the next difficulty level. Main Quests must be done sequentially and are unlocked only after completing the previous Main Quest/s.

Diablo IV Prying the Eye Walkthrough
The main quest Prying the Eye becomes available after completing Exhumed Relics where you're tasked to meet Lorath at the Forsaken Chapel. After the cutscene, speak to Taissa who's sitting by the couch to ask her how she's feeling. After learning that Lilith has made a deal with Astaroth and that she holds a key to hell, Lorath asks you to help him with setting up the Sigtheless Eye.
Lorath approaches the pedestal and asks you to bring the Sightless Eye which is just on the right side of the chapel, next to the chapel seats. Grab it and place it on the pedestal. Once it is on, Lorath asks you to gaze into Sightless Eye while picturing Lilith, allowing you to get in contact with Lilith.
Upon interacting with the Sightless Eye, a cutscene will trigger where you'll be able to peer into what Lilith is planning and who is with Elias. After a brief moment, Lilith senses your presence and sees you, which is only a matter of time before she comes after the group.
Lorath then explains about the dangers of Lilith consuming Mephisto's power, transforming her into a Prime Evil capable of conquering Hell and the world. Lorath then proposes an idea to stop her but in order to do so, you'll need to seek help from an old ally which you can find in Kyovashad. This completes the quest and you'll be rewarded with XP and Gold.

Taissa, is located in the Forsaken Chapel which is NE of Tarasak. The dungeon, Halls of the Damned, is to the SE from the Chapel.

Taissa, is located in the Forsaken Chappel which is in the NW of Kehjistan. The dungeon Tomb of the Saints is close by just SW.
I've finished the entirety of the chapter 3, but this quest did not start. I think it has more conditions than just completing Exhumed Relics. I was level 28 and had not yet fully completed any other chapter except the 3rd one.
