Alcarnus is a Stronghold in Diablo 4. Strongholds are heavily fortified locations in the overworld that have become occupied by demons. Clearing out Strongholds will leave a lasting impact on the world, creating safe havens for players, transforming into a new town and unlocking new Side Quests, Dungeons, Waypoints, shortcuts and more. Alcarnus is located in the Kehjistan region and is recommended for players of level 50 and above.

Diablo 4 Alcarnus Walkthrough
Upon entering Alcarnus, you will come across one or more Necrotic Masses, surrounded by lots of enemies. These masses' health bars are greyed out and you will not be able to damage them just yet. Instead, make your way to the Stronghold marker in the Lair of the Witch to the north.
In here, speak to the necromancer Rashta who is channeling a spell. She will initiate a fight which should be over fairly quickly, as once she falls to 10% health, she will shield herself and disappear into the Necrotic Mass in the room. Inspect the Medical Notes she leaves behind and then the three Research Notes marked on the map. This initiates the next objective which asks you to destroy one of the Necrotic Masses.
Return to the main area of the Stronghold and begin attacking the closest mass. Beware as you will be swarmed with a ton of enemies, including a Simulacrum of Rashta which you must destroy. Repeat the process for the remaining masses and simulacra to fill the progress bar and then return to the Lair of the Witch.
As you approach the Necrotic Mass in the Lair, Rashta will burst forth from it, reborn and more powerful than when initially encountered. She will use a combination of melee scythe attacks and magic.
Attacks to look out for:
- Rashta will release several projectiles straight up into the air which will fall in a ring formation in several waves around the target, starting with a very large ring at first, and contracting with every wave. Simply dodge out of the target zone in between waves. Note that the initial casting animation will damage players in a small radius around Rashta.
- Rashta will periodically absorb souls in the area and release them in a star pattern. This move is clearly telegraphed and can be avoided by standing in between each wave.
- Rashta will sometimes absorb blood from the surroundings and then burst, releasing projectiles that spiral outward from her for a brief moment. Simply step back from her during this time and avoid the projectiles' trajectory.
Periodically, Rashta will shield herself with blood and summon multiple Imperfect Simulacra. Defeat as many of the adds as soon as possible to avoid getting overwhelmed when the boss becomes vulnerable again.
Once Rashta is defeated, return to the entrance of her lair and activate the Wanderer's Shrine to conquer Alcarnus, liberating the Stronghold.
